This registration is designed to be filled out by a parent/guardian of students OR a leader attending the event. Confirmation will be sent to the designated registrar at the end of the form. If you need financial assistance, please click on the form button below and wait for your CHAOS leader to contact you BEFORE registering.
Note: There is a limited number of bus seats for skiing on Saturday, but unlimited spots for the lock-in on Friday night. If the registration is showing full, please fill out the wait list information. If another bus is added, you will be notified and can finish registering for skiing at that time. If you are only planning to attend the lock-in Friday night, fill out the form and you will be added from the wait list to finish registration.
